Health Notifications

To be in accordance with state of Michigan legal obligations, schools are required to notify and make available to parents and guardians of pupils (in at least grades 6, 9, and 12), information on immunizations, infectious diseases, or other school health issues, including information about meningococcal meningitis and human papillomavirus. This should be done at the beginning of the school year. The school notification shall include the following:

  • Causes and symptoms
  • How it is spread
  • Sources to obtain information about this contagious disease
  • Sources to obtain vaccinations to prevent the disease

The health notifications provided below can be included in your school communications to parents, posted on school websites, or on school bulletin boards, as well as printed as hand-outs in the school office or information centers.
